List of document required for loan issue

1. An application form for loan obtainment.

2. Registration documents, documents identifying the Client (for clients addressing the Bank for the first time) ¹:

2.1. Certificate on state registration of private individual – entrepreneur ^1,2;

2.2. Document confirming registration of private individual – entrepreneur ^1,2;

2.3. Depending on type of taxation:

• Document which verifies registration of private individual – entrepreneur at state tax authority (Form 4-ОPP) ^1,2 and 
• Certificate on single tax payment ^1,2, or
• Patent for  entrepreneurship under terms of fixed payment of income tax ^1,2, or
• Trade patent (specialized) ^1,2.

2.4. Passport or an equivalent identity document of private individual – entrepreneur;

2.5. Documents issued by state tax authority certifying assignment of tax payer identification number to private individual – entrepreneur (may not be provided if private individual did not receive such document in cases stipulated by the effective legislation of Ukraine) ^1,2.

3. Income declaration for four last reporting dates (originals or copies certified by the Client) or Statement of small business entity – private individual – single tax payer for four last reporting dates ².

4. Documents characterizing the subject of pledge and confirming property right for collateral ².

¹ Documents are not to be submitted if a Client has a current account opened at the Bank’s branch where the Client applied to for loan on condition that on the date of such application the updated package of documents is available in bank institution (that must be confirmed by a Client in writing).

² A document shall be provided in original form which shall be returned to a Client as soon as an authorized employee of the Bank makes copy of it and verifies its authenticity.
